毛友推介計劃 — 條款與細則
1. 推薦累積與獎勵
• 成功推薦3位新客戶的推薦人，將獲得價值HK$500的禮券（相等於2張HK$250的禮券）。
• 成功推薦5位新客戶的推薦人，將獲得價值HK$1,000的禮券（相等於4張HK$250的禮券）。
• 成功推薦10位或以上新客戶的推薦人，將獲得價值HK$2,000的禮券（相等於8張HK$250的禮券）。
• 成功推薦15位或以上新客戶的推薦人，將獲得價值HK$3,000的禮券（相等於12張HK$250的禮券）。
• 成功推薦20位或以上新客戶的推薦人，將獲得價值HK$4,000的禮券（相等於16張HK$250的禮券）。
• 成功推薦30位或以上新客戶的推薦人，將獲得價值HK$6,000的禮券（相等於24張HK$250的禮券）。
更高推薦數量的額外獎勵由城大動物醫療中心決定。
2. 禮券發放與使用
• 禮券將於新客戶完成註冊後的三個工作日內發放。
• 禮券將在下一次符合資格的付款時自動應用於帳單。
3. 禮券兌換與限制
• 每次帳單超過 HKD 1,500 的費用，僅可使用一張禮券。
• 每次帳單超過 HKD 3,000 或以上的費用，最多可使用兩張禮券。
4. 禮券有效期
• 禮券自發放之日起有效期為兩年。
• 禮券必須在有效期內使用，逾期未用的禮券將作廢。
5. 資格與限制
• 禮券不得轉讓，只能由推薦人本人使用。
• 禮券不能兌換現金或其他支付形式。
• 若新客戶符合其他折扣資格（例如員工優惠或城大學生優惠等），則僅適用較高的折扣，即新客戶不可同時享有多個折扣。
• 對於符合其他折扣資格的推薦人，例如員工優惠、城大教職員/學生/校友優惠或其他特殊身份優惠，扣除相關折扣後的最終費用可依照上述第3點所述的優惠辦理，前提是最後帳單金額需超過港幣1,500元或3,000元以兌換相關禮券。然而，此優惠不適用於購買任何“套餐服務”，如絕育服務、高齡猫犬保健計劃或愛寵尊尚計劃等。
6. 一般條款
• 推薦成功的定義為新客戶完成註冊後。
• 城大動物醫療中心保留核實推薦的權利，並有權取消任何欺詐或不善意的推薦。
• 城大動物醫療中心保留修改或終止本計劃或這些條款的權利，恕不另行通知。
7. 其他條款
• 城大動物醫療中心保留對這些條款的最終解釋權。
• 如有爭議，以城大動物醫療中心的決定為准。
8. 其他重要條款
• 推薦所有權：新客戶完成註冊後，推薦才被視為成功。
• 通知：推薦人一旦獎勵發放，將會收到電郵通知。
• 私隱：為計劃收集的個人資料僅用於行政目的，並遵守私隱政策。
• 責任：城大動物醫療中心不對因使用禮券或參與本計劃而造成的任何損失或損害負責。
• 不合規：任何濫用、欺詐或違反條款者，可能被取消資格或沒收獎勵。

PetPal Referral Program – Terms & Conditions
1. Referral Accumulation & Rewards
· Referrers who successfully refer 3 new clients will receive a $500 voucher (equivalent to 2 x $250 vouchers).
· Referrers who successfully refer 5 new clients will receive a $1,000 voucher (equivalent to 4 x $250 vouchers).
· Referrers who successfully refer 10 or more clients will receive a $2,000 voucher (equivalent to 8 x $250 vouchers).
· Referrers who successfully refer 15 or more clients will receive a $3,000 voucher (equivalent to 12 x $250 vouchers).
· Referrers who successfully refer 20 or more clients will receive a $4,000 voucher (equivalent to 16 x $250 vouchers).
· Referrers who successfully refer 30 or more clients will receive a $6,000 voucher (equivalent to 24 x $250 vouchers).
· With additional rewards for higher referral counts as determined by CityU VMC.
2. Voucher Issuance & Usage
· Vouchers will be issued three working days after the new client completes their registration.
· Vouchers will be automatically applied at the time of billing for the next eligible visit.
3. Voucher Redemption & Limitations
· For expenses exceeding HKD 1,500, only one voucher can be used per billing cycle.
· For expenses exceeding HKD 3,000, a maximum of two vouchers can be used per billing cycle.
4. Voucher Validity
· Vouchers are valid for two years from the date of issuance.
· Vouchers must be used within this validity period; any unused vouchers after expiry will be null and void.
5. Eligibility & Restrictions
· Vouchers are non-transferable and can only be used by the referrer.
· Vouchers cannot be exchanged for cash or any other form of payment.
· If New Clients are eligible for other discounts (such as staff discounts or CityUHK student discounts, etc.), only the higher discount will apply, and New Clients cannot enjoy multiple discounts simultaneously.
· For referrers who are eligible for other discounts, such as staff discounts, CityUHK staff/student/alumni discounts, or other special status benefits, the final fee after deducting the relevant discounts can be processed according to the benefits described in point 3 above, provided the final amount must exceeds HKD 1,500 or HKD 3,000 to redeem the voucher(s). However, this offer does not apply to the purchase of any “package services,” such as desex services, Senior Cats and Dogs Wellness Programme or Premier PetPlan, etc.
6. General Conditions
· A referral is considered successful only when the new client completes their registration.
· The clinic reserves the right to verify referrals and disqualify any that are fraudulent or made in bad faith.
· The clinic reserves the right to modify or terminate the program or these terms without prior notice.
7. Additional Terms
· CityU VMC reserves the right to interpret these terms and conditions.
· In case of disputes, the decision of CityU VMC shall be final.
8. Other Important Clauses
· Ownership of Referrals: Referrals are considered successful once the new client completes their registration.
· Notification: Referrers will be notified via email once their reward is issued.
· Privacy: Personal data collected for the program will be used solely for administration purposes and in accordance with privacy policies.
· Liability: CityU VMC shall not be liable for any loss or damages resulting from the use of vouchers or participation in the program.
· Non-Compliance: Any abuse, fraud, or non-compliance with these terms may result in disqualification or forfeiture of rewards.
